Bridges Outreach is a New Jersey nonprofit dedicated to ending homelessness and making homelessness rare, brief, and non-recurring. Bridges provides appointment-based case management services to individuals and families experiencing or at risk of homelessness throughout Essex, Union, and Somerset Counties.

Services may include housing navigation, connections to essential services and community resources, and Medicaid Housing Supports for eligible individuals. Our team works with each person to identify their needs, connect them with appropriate resources, and support their path toward housing stability and independence.
