Graham Home Services is committed to helping adults with disabilities live independently and with dignity. As part of the Graham Family of Services, alongside Graham Behavior Services, we bring individualized, person-centered support to adulthood, focused on independence, safety, and connection at home and in the community.

We work exclusively with New Jersey's Division of Developmental Disabilities (DDD), through both the Supports Program and the Community Care Program (CCP). Our team also helps individuals and families understand DDD eligibility, funding, and next steps, whether they are just beginning to explore services or have been receiving support for years.

Services we offer

• In-Home Supports
  One-to-one support with daily routines, personal care, household participation, safety, and independence at home.
• Community-Based Supports
  Support that helps individuals take part in community life, social activities, appointments, and meaningful experiences outside the home.
• Daily Living & Independence Skills
  Hands-on support with meal preparation, hygiene, organization, communication, and other skills that build confidence and capability day to day.
• Family & Care Team Communication
  Close collaboration with families, guardians, and care teams to keep services aligned and responsive to the individual.
• Transportation Support
  Assistance getting to appointments, community activities, and other important destinations.

Who we support
Graham Home Services is designed for adults with intellectual and developmental disabilities, along with the families, guardians, and support teams who help care for them. We support individuals who are already receiving DDD-funded services, as well as those who are just beginning to explore what support may be available.
