At Hearten, all of our services work together to promote independence in daily living, meaningful social connections, community integration, and personal growth and confidence. We provide a vibrant Day Habilitation Program in Hamilton, Mercer County, as well as Individual Supports and Community-Based Supports statewide. Our services are for adults aged 21+.

Our Individual Supports program serves all of NJ and provides personalized support at home or in the community. Individual Support service may include assistance with personal care routines, household organization, skill development, and community engagement utilizing individualized goal-driven plans.

The Community-Based Supports program (statewide) at Hearten is a flexible one-on-one support that is delivered in real-world community settings to strengthen independence and social integration. Services may include community participation and recreation, skill-building in daily routines, travel training and navigation support, and confidence-building through guided independence.

Hearten's Day Habilitation program (Hamilton, Mercer County) is designed to build skills, friendships, and confidence. Participants in our day program engage in social and communication development, money management and decision-making, technology and life skills training, health, fitness, and recreational activities, and community outings and volunteer opportunities.

All of our services are designed to complement each other, or they can be added as standalone services based on each individual's needs.
