LilyPADS (Pregnancy Access for Diabetes Support) offers free telehealth diabetes support to pregnant patients with gestational or pre-existing diabetes who live in qualifying NJ ZIP codes. The program is funded through the New Jersey Rural Health Transformation Program and run by LilyLink.
Eligible patients receive:
• A free continuous glucose monitor (CGM) or glucometer shipped to their home, with a setup and training call
• Video visits with a registered dietitian and certified diabetes educator for diabetes education and meal planning built around foods they already eat
• Weekly glucose review with maternal-fetal medicine (MFM) input, through delivery
LilyPADS supplements a patient's regular prenatal care and does not replace their OB or midwife. The patient's prenatal provider receives regular updates, including glucose patterns and any MFM recommendations.
Who can enroll: Pregnant patients with gestational or pre-existing diabetes who live in qualifying ZIP codes in Atlantic, Burlington, Cape May, Cumberland, Hunterdon, Mercer, Monmouth, Ocean, Salem, Sussex, and Warren counties. Insurance status doesn't affect eligibility, so uninsured and underinsured patients are covered.
How to enroll: Patients can self-refer, or their prenatal provider can refer them.
Cost: Free for patients in qualifying ZIP codes.
