• Rapid Access: Initial welcome calls are available immediately, with intake appointments typically scheduled within the same week.
• High Engagement: Patients average over 70 touchpoints with their dedicated care team during the first month.
• Flexibility: We offer appointments seven days a week, including evenings and weekends, to accommodate work and childcare responsibilities.
• Insurance & Coverage: We accept Medicaid, Medicare, and commercial plans, and offer a self-pay option of $245 per month.

How to Refer
Referring a patient takes less than one minute via our website. We only require:

• Patient’s name, date of birth, and phone number
• Your contact information

Once submitted, our enrollment team manages the entire onboarding process, including the welcome call and insurance verification.
